En renografi är en vanlig nuklearmedicinsk undersökning av njurarnas funktion med hjälp av ett radioaktivt ämne som injiceras intravenöst. Ibland behöver det tillsättas ett vätskedrivande läkemedel i undersökningen för att kunna skilja på obstruktivitet och en långsam tömningshastighet. En systematisk litteraturöversikt ska göras för att få djupare information kring när, i förhållande till radiofarmakan, som det vätskedrivande läkemedlet ska ges i undersökningen samt vad optimal dosering av det vätskedrivande läkemedlet vid renografier är.

A dynamic renography is a common noninvasive examination where the split renal function can be examined or obstructive uropathy detected. A diuretic is administered in some renographies to distinguish between obstruction and a prolonged renal excretion. A systematic litterature review will be performed to get a deeper understanding regarding when the diuretics should be administered in relation to the radiopharmaceutical during the renography and to discover the optimal dose of diuretics in a renography.

A dynamic renography is a common noninvasive examination where the split renal function can be examined or obstructive uropathy detected. A diuretic is administered in some renographies to distinguish between obstruction and a prolonged renal excretion. There is a large variability in how a renography is performed among Nuclear Medicine departments as there is no gold standard technique. The variability is mainly as to the timing of the diuretic administration during the renography. The timing of the diuretic given after the radiopharmaceutical is described as F+ and before the radiopharmaceuticals as F-. The F+20 method, i.e. diuretic given twenty minutes after the radiopharmaceutical, has been the method considered to be the traditional method. Optimal diuretic dosage is a challenging issue in diagnostic imaging, for example in a renography. The present Nuclear Medicine department in Örebro administer a diuretic, Furosemide, based on the patient’s weight. It is administered ten minutes into the examination. The standard administration dose used is 0.5 mg/Kg with a maximum dose of 40 mg, which is administered to the majority of our patients as well as in many other Nuclear Medicine departments. Furosemide is also sometimes used when the urinary tract is examined in a Computed Tomography. However, the dose is than much lower than when examining the urinary tract in a gamma camera. Yet, the purpose of the Furosemide administration is the same, to achieve adequate diuretic effect, regardless of diagnostic modality. The present Nuclear Medicine department does not aim to administer diuretic in vain, instead with an optimal dose and only when necessary. A systematic literature review of the timing and the dosage of Furosemide administration in 99mTc-MAG3 renographies will be performed. The study will be registered in the Prospero database. A meta-analysis will be performed if possible, otherwise a narrative synthesis. The method will follow the PRISMA method (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). At least two databases will be searched for primary articles and published guidelines together with an information specialist. The PICO method will be used in two different searches.1.
