Andfåddhet är en vanlig symtom i primärvården and är en vanlig orsak bakom otaliga sjukhusinläggningar. Olika mätmetoder används i mätningen av andfåddhet och en nehetlig metod som kan beskriva flera olika dimensioner av andfåddhet behövs för att underlätta identifiering och uppföljning av dessa patienter.
Aim: This study aimed to compare and correlate four common but different measurement tools and relate them to the quality of life of patients presenting with dyspnea. Methods: The study was a cross-sectional study conducted in primary care targeting dyspnea as a symptom entity. One-dimensional scales used in the study were: New York Heart Association (NYHA) functional classification scale and modified Medical Research Council (mMRC) aimed to measure impact of dyspnea on physical functionality. The intensity of dyspnea is measured by Visual Analogue Scale (VAS) and Verbal Rating Scale (VRS). The questionnaire used to determine health-related quality of life (HRQoL) was SF-36. Results: In total, 89 patients were included in the analysis. The mean age of the participants was 69 ± 9.7 years, and 49.4% were female. All items in SF-36 showed high internal consistency. The HRQoL among patients with perceived dyspnea was significantly reduced shown both by SF-36 and one-dimensional scales. The multidimensionality of dyspnea was confirmed by both type of one- and multidimensional generic scales. The most responsive rating instrument shoved to be mMRC, which provided a significant positive association with NYHA (r = 0.67, p<0.001). Conclusions: A combination of both generic and one-dimensional rating scale is a suitable approach to quantify dyspnea and evaluate the impact of dyspnea in HRQoL in patients seeking medical advice complaining of breathlessness in primary care.
