Symtom contextualisation in primary health care interactions

In primary health care, 20-30% of patients present with symptoms with no demonstrable cause, ie symptoms that are "functional". Common examples of symptoms are dizziness, fatigue, dyspepsia, myalgia and joint pain. These symptoms are commonly as disabling as those with an organic etiology and patients are more often female and socioeconomically deprived. Several types of treatment (eg pharmacological, behavioural and physiotherapeutic) have been tried but met with little success.

Previous research has indicated that the communication between these patients and their health care providers is of paramount importance. We therefore plan to study the way patients with functional symptoms communicate with health care staff, in order to shed light on how symptoms are interpreted in interaction. Suitable participants will be selected by screening medical case notes among patients who call their health care centre for advice. Audio-recordings will be made of telephone consultations with nurses and face-to-face consultations with physicians, as well as of interviews. Data will be analysed using discourse analysis and computer linguistic methods.

This will hopefully yield new ways of approaching the problem, suggestions for structured health-care plans (algorithms) and methods for early identification of patients with incipient functional disorders. Results will be useful for designing clinical trials aimed at reducing the burden of functional symptoms for patients and society.
